Doctors operating on a 67-year-old man for a ‘hernia’ were taken aback when they discovered a testicle and feminine genitalia inside the bulge.

The father of three, who was born with only one descending testicle, went to a hospital in Kosovo complaining of a decade-long swelling in his groin.

Doctors discovered uterus, cervix, fallopian tube, and ovary, as well as his missing testicle, when they examined the 15-centimetre-by-10-centimetre protrusion.

The individual was diagnosed with Persistent Mullerian Duct Syndrome (PMDS), an exceptionally rare disorder in which men’s penis and scrotum are joined by female reproductive organs.

Medics from the University of Prishtina published the case report in the journal Urology Case Reports.

The patient had a lump for ten years that grew larger when he stood up, coughed, or had abdominal tension, but flattened when he lay flat, according to the study.

An inguinal hernia is a condition in which tissue from the top of the inner thigh pokes through into the groin. According to the Daily Mail, it is the most common type of hernia and is mostly seen in men.

When the physicians operated on him, however, they discovered a “pear-like object” that turned out to be a uterus. A “scrotal sac” was also present, containing a fallopian tube and a testicle with an ovary attached.

The patient was diagnosed with PMDS, which has an inguinal hernia as its primary symptom.

While people with PMDS are at risk of infertility, the 67-year-old patient had three children and a “well developed penis” with a urethral opening in the proper location, according to the doctors.

In the medical literature, only 200 cases of PMDS have been reported.
